What can you expect through this process?
- Complete the above application
- Fallen Fawn reviews and approves application
- Receive an approval letter via email with a list of approved providers
- You will contact a provider from the approved provider list
- All scheduling will be done with the providers office, no information will be shared with Fallen Fawn Foundation
- Invoices will be sent from the provider to Fallen Fawn Foundation to be paid for
- After a few months of services you will receive a short satisfaction survey with a chance to include an anonymous testimonial.
